Reception will email a questionnaire when you schedule your initial appointment.

What to bring:

1. Completed New Patient Questionnaire:

Please fill in all sections. This gives your practitioner important information about your current health concerns as well as your health history.

If this form is not complete prior to your visit we will have to spend consultation time to do so.


2. Any recent test results/ Pathology reports/ imaging reports and films if available

Bloods, Stool, Urine, Scans, X-ray reports, other.


3. List of current prescribed medications and nutritional supplements:
